Points earned on qualifying products are added to your ZEISS Practice Advantage account approximately 7-10 days from month end. Non-ZEISS labs points may take up 60 days to be added to your account.
Points can be redeemed for the options, below:
A completed IRS W-9 tax form must be on file with the ZEISS Practice Advantage Department before any point can be redeemed. The IRS W-9 tax form is available for download here Instructions are on the form provided.
Points accrue based on the calendar year and expire 12-months after year end. Points earned between January 1st - December 31st, 2016 will expire in early December 2017. Points earned between January 1st - December 31st, 2017 will expire in early December 2018.
Points can be redeemed for the options, below:
- Direct Rewards: A check redeemed at the standard $0.08/point conversion rate
- Merchandise: ZEISS Sport Optics, ZEISS branded cleaners, merchandise and other products available at www.partner.zeiss.com.
- Equipment/Service Claims: Participants can setup a claim to receive automated checks for practice building purchases, on a 4-week or quarterly cycle at $0.10/point.
